Limited Time 6-Pack Discount: I am offering all new AND existing self-pay patients a 10% discount on the purchase of a 6-pack of follow up visits if purchased by April 30th, 2020 (at least one visit must be used by May 15th, 2020).

You may use your HSA (Health Savings) /FSA (Flex Savings) account to make payments for physical therapy services, whether you are in-network, out-of-network, or self-pay.

We are also happy to provide a Superbill for you to submit to your insurance company for reimbursement if using out-of-network benefits.

This form is also available at our office and you’re welcome to complete it just before your appointment if that is more convenient. Please arrive 10 minutes before your appointment to complete the form at our office.

What should I expect on my first visit?

You will be meeting one-on-one with an experienced, licensed Doctor of Physical Therapy during your first visit to Physical Therapy Evolution.

We will take the time to gather your medical history and conduct a thorough assessment and analysis of your injury or condition. Finally, we will recommend and discuss your course of care and start your first treatment.

Please wear or bring comfortable, loose clothing that does not inhibit your body’s ability to move and stretch.

Please bring your referral from your physician (if required by your insurance), any imaging reports you may have, and a list of any prescribed medications as well as over-the-counter medications you take regularly.

Do you accept insurance?

We accept most major insurances and cash payments. Please see listing above for accepted insurers.

Do I need insurance to be treated?

No, we accept cash payments for services.

What if I don’t have insurance?

You’re welcome to self-pay for your treatment. Our rates are listed above.

Do I need a referral from my doctor to see you?

It depends on your insurance plan. We are a direct access state so you may call and book an appointment with us without a direct referral from your doctor. However, some insurances do require a referral before your first treatment.

How long will my treatments take on each visit?

Please allow 55 minutes for your initial evaluation and treatment. Follow-up visits will be 40 minutes in length.

How many visits will I need?

During your first visit, we will advise you of the recommended frequency and duration of visits needed. This will depend on many factors including the chronicity and irritability of your problem, and any recommendations from your physician.

We know that patients often want more insight to their injury. You are always welcome to come in for a consultation and we will be glad to answer your questions. However, should you want to do some research on your own, we’ve found the information available on following websites very educational and accurate.
